Since we all know pretty much since it's both in the bio and in the first chapter I'm gonna start off with this he got his from Izumi when she accidentally had a knife and her giving him this was the final straw after getting brand the same day he ran away and made his tormentors realize that they done nothing but cause abuse to him.

The scars that he got on his body came from his tormentors as Momo found them sexy that lead to them having their first time having sex at the age of 17 (which they are 20 now and that UA is a college not a highschool).

Most of them came from Inko as she whipped him because of Izumi lies of her saying that he bullied her knowing that he didn't at all.
Tattoos
Izuku has a tattoo on his left wrist with the Numerical of IX•XXIII it's a sign of her birthday and that he asked her on the same day on her 16th birthday which Loid and Yor accepted him after they heard about his past. Also Yor wanted to be Izukus mother after she heard about what Inko did to him which they realize he began to cry and they all cheered him up.
Izuku has a tatto of his x her name on his torso because after he saw that Momo stayed by his side even if he got misdiagnosed she treated him as an equal as of he was quirkless.
